	CSM Paper Application	If you prefer to use a paper application, you may call (888) 446-9489 or email the Admissions Office.
	You will need to complete the Supplemental Forms section of the online application, which includes: The International Student Fact Sheet A Statement of Support TOEFL scores or Proof of English information
In addition, you will need to send:
	Official transcripts (grade reports in secondary school, college, or university), certificates, and/or diploma demontrating completion of preparatory studies for higher education.
	Certified literal English translations of all transcripts and supporting documents that are in languages other than English.
	List of courses in which you are currently enrolled.
	Supporting documents or letters from bank or sponsoring agency.
	$45 Evaluation Fee Note: If you pay the application fee on-line, it will be transmitted automatically to CSM Admissions. If you pay by check, please make your check payable to CSM Admissions.
