Health Insurance

Health insurance coverage is mandatory for all international students enrolled at Colorado School of Mines, regardless of their course load.
 
Insurance coverage will be automatically included in international student tuition bills and students will no longer be required to make separate payment arrangements.
 
Students will still be free to choose alternative insurance as long as their plan's coverage meets or exceeds the minimum requirements listed at the bottom of this page. In order to use alternate insurance, students must complete and submit waiver request forms no later than Sept. 9, 2009. If a waiver request is submitted and approved before Sept. 9, the insurance charge will be removed from the tuition bill before it is printed. Any request that is submitted after September will not be approved and the student must pay in full (typically due by September).
 
The intent of the insurance requirement for international students at Colorado School of Mines is continuous coverage during and between academic terms.

Students who wish to waive the insurance must have a plan that meets or exceeds the requirements below.
The lifetime maximum benefit is at least $1 million (with no yearly or per condition maximum benefit that would reduce coverage).
  • The plan includes participating health care providers (i.e. hospitals, physicians, pharmacies, and mental health care providers) in the Denver metro area for both emergency AND non-emergency health care services.
  • The plan includes prescription drug benefits.
  • The plan provides at least 20 outpatient visits for mental health care services and provides at least 30 days of inpatient mental health care services (including emergency psychiatric admissions).
  • Coverage will be in effect as of Jan. 7, 2009, without any waiting period or pre-existing condition exclusion.
  • The plan will remain in effect for the 08-09 academic year, except for termination due to attainment of a maximum age or other condition resulting in loss of plan eligibility.
  • The plan does not have a deductible greater than $500 (if the deductible is greater than $500, you must check the following statement confirming that you have adequate financial resources to pay for medical expenses subject to the deductible). 
    • I have adequate financial resources to pay for any medical expenses that are subject to a plan deductible that is greater than $500.
  • If you travel abroad during the 08-09 academic year as part of a CSM program, the plan must provide medical evacuation and repatriation coverage at a level comparable to the SHIP. If my personal insurance does not have this coverage, I will purchase other individual insurance coverage that provides comparable medical evacuation and repatriation benefits.
