Student Rights and Responsibilities
We understand that achieving your academic potential is critical to being a successful student at CSM, and are committed to providing equal access to our education programs. CSM welcomes applications from qualified students with disabilities and assures them that CSM will provide access to all programs for which they are qualified.
OSSD coordinates CSM’s efforts to comply with the broad mandates expressed in Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990.
Although a student with a disability does not have to self-disclose or register with the Office of Services for Students with Disabilities, we encourage them to do so. A student seeking academic accommodations or adjustments must contact our office to request services.

• Provide current documentation from an appropriate health care professional, that establishes the existence and current (preferably in the past 3 years) status of a disability and supports the accommodations requested, including:
We will review the materials and make a decision regarding appropriate academic accommodations.. These determinations are made on a case-by-case basis.
Once eligibility is determined, it is important that the student follow established policies of the OSSD in regards to timeliness of requests and completing necessary forms in order to properly coordinate accommodations. The accommodation process is an interactive one and the student is expected to help make sure that the process works.
Students must renew their request for services with the OSSD at the beginning of each school year.
• Appropriate confidentiality of all information related to your disability.
• Equal access to courses, programs, services, jobs, activities, and facilities available through the University.
• Appropriate reasonable accommodations according to your disability, which may include academic adjustment, and/or auxiliary aidsdetermined on an individualized basis using required documentation.
• Be treated respectfully by all University personnel.
• Disclosing your disability and providing current documentation to the OSSD. Please make an appointment with OSSD prior to your first semester of attendance to discuss your disability, provide current documentation, and request for accommodation.
• Completing the Accommodation Request Form and meeting with the OSSD staff member each semester to review course requirements and to obtain Confidential Faculty Letters for each instructor.
• Meeting with faculty to discuss disability-related needs and accommodations.
• Requesting all services in a timely manner.
• Reporting any concerns that you have regarding accommodations as they arise. Otherwise, it inhibits the OSSD’s ability to address your concerns.
• Treating all university personnel with respect and adhere to the institution’s standards of student conduct.
• Meeting the University’s graduation requirements and qualifications essential for full participation in any academic program.
