Graduate Assistantship, Hourly and Fellowship Appointment Materials

2007-2008 Calendar Year

The following are implementation forms for graduate assistantship, graduate hourly and graduate fellowship appointments.

Student Agreements must be submitted by the appointing department/division at least once per calendar year per student. This completed form represents the formal agreement entered into by CSM through the department/division with the student. It should include all information that spells out for the student appointee the compensation to be received for the specific services requested.


The appropriate form should be uploaded and completed by the appointing department/division. Once completed, it should be signed by the appropriate departmental representatives and the student. Originals of the completed and signed form should reside in the department/division with a copy sent by campus mail to Human Resources.

Institutional Implementation Commitments must be submitted by the appointing department/division once every semester. This spreadsheet specifies, in detail, how CSM is to financially implement the student appointee agreement defined by the Student Agreement completed above.

For each student agreement, a separate spreadsheet should be completed by the department/division and emailed to Human Resources at student.contracts@is.mines.edu.

As Graduate Fellowship Awards are made without the expectation of services being rendered, departments/divisions do not need to complete a Student Agreement form to implement a graduate fellowship award.
