Summer Undergraduate Fee WaiverUndergraduate students who are enrolled in only online courses and/or field session courses that have no on-campus component, as determined by Colorado School of Mines, are eligible to request a fee waiver for all mandatory student fees except technology fee.

Part-Time Grad Fee Waiver – Graduate students who are enrolled in six or less credit hours for a semester may be eligible to waive a portion of their mandatory fees.

Submission for Individual Taxpayer Identification Number (ITIN) – This form is used to submit your ITIN or SSN number. This form is secured and only authorized employees of the Office of the Bursar at Colorado School of Mines has the access to view the information submitted.

Financial Hold Transcript RequestIn accordance with Colorado HB 22-1049, a student who has a financial hold but is requesting for a transcript and/or diploma must complete this form.


Sponsor Authorization – This authorization form provides the ability to sponsors to select specific student charges that they are willing to pay (e.g. tuition, fees, room, board, health insurance, etc).

Please note: Financial Guarantee Letters are not accepted for billing purposes.

Sponsored Student Agreement – This agreement form grants the Bursar Office to invoice and provide student information to their sponsor. Information may include, but is not limited to, tuition and fee charges, grades, and financial award amounts.

Campus related Rentals

Cash Box Rental – This form is required for all cashbox rental for campus-related events.

Card Terminal Rental – This form is required for all card terminal rental for campus-related events. All transactions made with the card terminal will be assessed a non-refundable card convenience fee to the index/account provided for deposit.

Faculty and Staff

CASHNet Access – Departments who receive payments will need CASHNet access to create departmental deposits for these payments.

Employee Tuition Waiver – Faculty and Staff may be eligible to enroll in six credit hours of coursework each academic year and have their tuition waived. Employees will be responsible for any fees assessed to their account based on their enrollment for the specified semester.

Student Award – This form allows departments to provide financial grants/awards to students from their index. Student Award Forms can be submitted through OnBase (Onbase > Open Workflow > AR3-Student Award Workflow). 

Card Authorization – The Bursar’s Office will no longer accept card numbers from the department for deposit processing. For more information and options on card payments, please click on the link here.
