CLERY TRAVEL FORM

As a higher education institution, Mines must comply with the Jeanne Clery Act. The Clery Act is a federal law that requires colleges and universities to gather and disclose crime statistics that occur within Mines’ Clery reportable geography, which includes qualifying off-campus student activities and travel. Any off-campus activity that meets the criteria in the Clery Act, must be reported to Mines Clery Compliance Office, regardless if any reportable crimes were known to have occurred. WHAT IS CONSIDERED AS MINES' STUDENT TRAVEL?
  • Over-night, school-sponsored trips at which Mines paid for the lodging of the student or rented a space for student to use.
    • Example: Mines graduate student goes to Boston for a conference and Mines pays for their lodging
    • Note: Trips that are paid with grant funds must also be submitted
INFORMATION REQUIRED

Please note:

  • It is required that Student Travel Forms be submitted either before or immediately after travel has taken place
  • To ensure that the Clery Act required information is properly collected and stored, the form must be used. Email submissions cannot be accepted
  • Please do not fill out a form if the travel information is not yet known. Hotel rooms/units do not have to be disclosed if filling out the form before travel

    • The student who attended the trip or;
    • the student or Faculty/Staff member leading the group (if more than one student attended)

  • Hotel/Travel Address
  • ENTER THE ADDRESS THAT THE STUDENT(S) WILL BE TRAVELING TO/STAYING AT. DO NOT FILL OUT USING A MINES ADDRESS OR YOUR HOME ADDRESS.
  • Optional: Any additional address that may be used during the trip

Note: Only one entry is needed per trip regardless of how many students attended
