This is a reminder that Colorado School of Mines requires all faculty, staff, and students to submit proof of COVID-19 vaccination or a vaccine exemption form by Monday, August 23.

Please check www.mines.edu/vaccine for more information, including instructions for how to submit proof of vaccination or exemption form.

It is important that we obtain this information so that Mines can make informed decisions in response to evolving COVID-19 impacts on our community.

Please remember that the decision to implement and enforce a vaccine requirement was made after careful consideration of input from our community. We also continue to strongly recommend getting vaccinated as the most effective way to protect yourself and others in our community from potential adverse health effects of COVID-19.

If you still have not received your vaccine, Mines will be hosting vaccine clinics on campus August 23 and 27.

All three FDA-authorized vaccines will be available at the clinics.
