Degree Requirements – Degree Audit Form

All courses required for a degree must be listed on the Degree Audit form.

  • The Office of Graduate Studies (OGS) only enforces institutional minimum courses/research requirements, not departmental requirements.
    • Signature on Degree Audit form by advisor/committee/department head indicates that degree should be awarded based on the coursework/credits listed and any other departmental requirements have been waived.
  • Courses to be listed on the Degree Audit form
    • Any course/credit required to earn the degree
    • Unlimited graduate level credits (500, 600 or 700) taken at CSM while a graduate student
    • Maximum of 9 credits of 400 level coursework
    • Maximum of 6 credits of double counted coursework.
      • Applicable only to combined students in programs that allow students to double count .
    • Transfer credits (limits apply – see transfer limits)
      • Transfer credits from other universities:
        • OGS must have official transcripts
        • List in the transfer section on Degree Audit
      • Transfer credits from CSM – taken while an undergraduate
        • Do no list these credits in transfer section. Instead, list in Section A.
        • Credits cannot have been used towards the undergraduate degree.
      • PHD students using CSM graduate level work taken while earning a CSM Master’s degree ARE NOT transfer credits. List all these credits in Section A.
  • Responsible Conduct of Research
    • All thesis based students must check the appropriate box.
    • Any student receiving NSF funding must take one of the Responsible Conduct of Research courses. NSF funded students who do not complete the coursework will not be awarded a degree.
  • Credits required to earn the degree must match the total number of credits listed on the Degree Audit form.
    • List the credits you have already taken and any credits you plan to take in the future so the total number credits listed equals the number of credits required. In the event that you list a course to be taken in the future and you do not  take that course, you need to complete an Addendum to Degree Audit form.
