The Fee Waiver Request form initiates a request from a part-time student to waive some of the mandatory student fees. To make this request, please download the Fee Waiver Request Form

You should print this form, complete it,  and send to your advisor and department head/division director for their signatures. Once your advisor and department head/division director have signed, you may submit the signed copy to the Bursar’s Office, Student Center, E172.

This request MUST be submitted by the Last Day to Register for the Fall or Spring semester. Please submit this form to the Bursar’s Office. Requests submitted to the Bursar’s Office after that date will NOT be considered.

This request MUST be submitted each Fall and Spring semester.

Note: Students on Reduced Registration are considered full-time and therefore are NOT eligible for any waivers of the mandatory fees.
