Benefit eligible Faculty/Staff and Classified Staff are welcome to participate!

SERVICES BEING OFFERED*

  • The Company Code for the Life Health website is MINES2021

  • Biometric Screening (Blood Testing).   

  • Flu shots – Faculty/Staff, please bring your Medical ID.

  • Classified Staff – will be charged a minimal

    • Homocysteine
    • Cardio Reactive Protein (CRP)
