New Employee Information

Welcome to Mines!

We are glad you have made the choice to join the Mines community. This page will provide you with links to the required new hire forms that will allow us to complete the hiring process.  Please select the correct new hire packet for your employment type. 

Please print and complete forms prior to your first visit to the Controller’s Office/Payroll Department.  Faculty and staff should read and become familiar with the school policies available below on the Policies link.

 

Original Social Security Card Required

  • Original Social Security Card: Mines is required by state and IRS guidelines to pay employees by the name that is printed on the original security card. If you do not have a card or need a replacement, please contact the Social Security Administration for information. Mines policy allows us to pay one month without obtaining a copy of your social security card.  The SSA receipt of application will serve as proof that a card has been requested and will allow HR to enter your information for payroll.  Upon receipt of the actual card, please bring the original to the HR department.
  • Directions to the nearest Social Security Administration office in nearby Lakewood, Colorado.

STUDENT EMPLOYEES

STUDENT NEW HIRE PACKET: Please print out, sign all forms, and bring to the Controller’s Office/Payroll Department with your original I-9 documents and your social security card no later than your first day of employment.

International students: you also need to bring your original immigration documents no later than your first day of employment.

  • Undergraduate Employment/Work-Study
  • Student TIAA Benefits: TIAA is a qualified retirement plan that substitutes for Social Security, as required by law.  Because Mines is not a Social Security covered employer, student employees who are not enrolled in classes at least half-time during the semester must participate in TIAA.

INTERNATIONAL EMPLOYEES AND STUDENTS

In addition to the new hire packets, all international employees and students must also provide the following documentation:

  • Work Authorization Letter from the CSM International Office – J1 and F1 visas
  • I-94:  Arrival/Departure Record
  • I-20 or DS-2019
  • Visa/Passport
THE CLERY ACT AND CAMPUS SECURITY REPORT

The Colorado School of Mines Annual Campus Security and Fire Safety Report contains valuable information for all campus community members. The report is designed to inform you of campus crime and fire statistics and provide security policy statements as required by the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (the Clery Act), a federal law requiring institutions of higher education to annually disclose this information.
