500-Level Registration Form 500-Level Registration Form All undergraduate students (students who have not yet graduated from their BS program) must submit this form for approval to enroll in any 500-level (or above) graduate-designated course. If approved, students will be administratively registered for the requested course (given course availability). Use this form to indicate whether the requested course should apply toward your BS degree only, be a double-counted course (as part of the Combined/4+1 program), or toward your Graduate degree only. Note that registration changes to any 500-level course must be made by the add/drop date of the term in which the course is taken--no exceptions. Students may count ONLY six 500-level credits toward their BS degree (which may be double-counted between BS degree and Graduate Degree for Combined students). Any additional 500-level courses must appear on the graduate transcript only.
