  • Proof of Dissemination of Research

    To prove dissemination of research, you can EITHER 1) provide a link to a journal, booklet, or other document containing your name, co-authors names, title of research project, and the year presented/published OR 2) complete the downloadable Proof of Dissemination of Research Form found under "Needed Materials for Applying" (on the previous page) and upload it under “2. Proof of Dissemination of Research Form ..."

  • Research Mentor Recommendation

    When you submit your application, a link to a recommendation form will automatically be sent to the faculty mentor you list below. Please double check you have not misspelled your faculty mentor's name and email address.
  • Semester #1 of Research Experience

    To be eligible for this distinction, you must complete at least two semesters of undergraduate research (totaling no less than 120 hours of research or approx. 4-5 hours of research per week, per semester). It is preferred that both semesters be conducted at CSM on the same project with the same mentor. Ideally, the mentor you listed above, will also be listed here. There is flexibility with this requirement. Please fill out the below information with your most recent Mines research experience. *Please note that research undertaken through senior capstone, senior design, or another required course does not fulfill this requirement. Research taken for credit through independent study credits likely will meet the requirement.
  • Semester #2 Research Experience

    This section is a continuation of the above section. Only fill 2.a. through 2.g. IF this research project is different than the research project you listed in 1.a. through 1.g.
  • More About You ...

    If you are selected to receive Undergraduate Research Scholar Distinction at graduation, this portion of the form will be used by the office of Undergraduate Research Scholars to mail you a silver cord before graduation (if you are remote) and create a webpage recognizing your success.
  • Max. file size: 250 MB.
  • Your bio should be 3-6 sentences in THIRD PERSON, PAST TENSE (e.g. Jane Doe graduated from Colorado School of Mines with a B.S. in Biochemical Engineering ...). Your bio should include 1) your major and minor, 2) your research department, 3) any other honors or awards you received during your undergraduate tenure, 4) a bit about the research you’ve done, and 5) a bit about what you would like to do in your future.
