Report COVID-19

Mines students, staff, faculty and contractors, please use this reporting form if you:

• Have received a positive COVID-19 test result
• Have had close contact (live with or had extended face-to-face contact for >15 minutes within 6 feet) with someone who has tested positive for COVID-19 (irrespective of mask use)
• Have been tested for COVID-19 and are awaiting test results

These reports are received by the Mines COVID-19 Response Initiator Team (CRIT) seven days a week between 7:30 a.m. and 6 p.m. Reports received outside these hours will be addressed the following day.

The following questions are not required but providing answers here will help Mines respond and intervene more quickly on confirmed/presumed cases of COVID-19 within our campus community. If you do not feel comfortable submitting this information via the form, it’s OK to leave it blank. A member of the Mines COVID-19 Response Initiator Team (CRIT) will be in touch shortly to gather relevant information directly.

This form is submitted to the Mines COVID-19 Response Initiator Team who will protect your information and share it with only those with a legitimate public health need. CRIT will inform the Jefferson County Public Health department so they may take appropriate actions as required by the State of Colorado.
