I, the parent or legal guardian desire to enroll my child, in SUMMET 2023 (Summer Multicultural Engineering Training Program). I understand that this is a summer program (hereinafter “SUMMET 2024”) for high school students sponsored by Colorado School of Mines (“Mines”). SUMMET 2024 will be held June 2 – June 7, 2024, June 9 – June 14, 2024 and June 23 – June 28, 2024 at Mines campus in Golden, Colorado.

I understand and agree that I am responsible for transporting my child to and from Mines at the beginning and end of SUMMET 2024, respectively. Most of SUMMET 2024 activities will take place in classrooms, computer laboratories and other facilities on Mines campus, although off-campus events will be held. The off-campus events will include social programming (such as bowling, mini-golf, Rockies baseball game, etc.) and industry field trips to Schlumberger, IBM, or Lockheed Martin. Off-campus events will be chaperoned by SUMMET staff and mentors, and travel will occur in Mines owned vans/shuttle buses.

I understand that SUMMET 2024 activities will include instruction and hands-on training in basic science and engineering courses. Participants may also engage in some recreational activities and games involving a moderate level of physical activity.

In consideration for the opportunity to enroll my child in SUMMET 2024, I hereby voluntarily assume full responsibility for all risks of loss, property damage or personal injury, including death that may be sustained by me or by my child as a result of my child’s participation in the Program. I agree to HOLD HARMLESS, RELEASE INDEMNIFY AND FOREVER DISCHARGE the State of Colorado, Mines, and Mines Board of Trustees, directors, officers, agents, employees and representatives from any and all claims, demands, and causes of action, of whatever kind of nature, either in law or equity, which arise from, or are connected in any way with my child’s participation in the SUMMET Program.

If emergency medical action or treatment is required for my child and neither parent nor guardian can be contacted, I hereby consent to my child being administered medical care by the physician or licensed hospital or medical care facility deemed by the Mines representative in charge to be the most expedient and appropriate under the circumstances.

Nothing in this Parental Consent, Liability Waiver and Release Form shall be construed to waive, limit, or otherwise modify any governmental immunity that may be available to Mines trustees, officers, employees, agents and representatives under the Colorado Governmental Immunity Act, Colorado Revised Statutes  § 24-10-101, et seq.

Every term and provision of this agreement is intended to be severable. If any portion hereof is found to be invalid or unenforceable, the remaining terms and provisions shall continue in full legal force and effect.
